Hospital Based Nursing Facility
CHRISTUS Spohn has a specialized Hospital Based Nursing Facility (HBNF) located within CHRISTUS Spohn Hospital Corpus Christi—Shoreline. The goal of the HBNF is to provide our patients with the opportunity to achieve and maintain their full function after a debilitating illness or condition.
Patients entering the unit must have been admitted to the hospital a full three days prior to being transferred to the HBNF and have skilled needs as defined by Medicare—including IV or tube feeding, tracheotomy care, IV therapy, rehabilitation needs or would care. A physician’s order is required for admission into the HBNF.
Patients who are candidates for admission to the HBNF fall into three categories:
- Patients who cannot tolerate an inpatient acute rehabilitation program;
- Patients who have complex medical diagnoses that require a skilled level of nursing and therapy care; and
- Patients who require additional rehabilitation prior to their return home, but do not require the intensity of therapy provided on the Inpatient Rehab unit.
Patients requiring physical rehab on the HBNF receive a program designed specifically for their individual needs from occupational, physical, and/or speech therapy. Patient care in the HBNF is directed by a physician and offers an individualized interdisciplinary approach to patient care.
